FVO — Formal Verification Online
Lesson 11.5 · Formal Signoff on Real Projects

The signoff report

The signoff review is short if the plan was good. A one-page summary per block typically contains:

  1. Scope: configurations, RTL revision, tool and version.
  2. Results: assertions proven / bounded (with depth and justification) / waived (with reason and owner).
  3. Environment: assumptions with their references and where each is asserted.
  4. Quality: covers (all reachable, max depth), vacuity, mutation summary, COI/proof-core coverage with explained gaps.
  5. Bugs found: count, severity, and which were found by formal only.
  6. Open risks: anything the next level of verification must cover.
